Little to no oversight exists and management fails to acknowledge or own the problems. For example, when a former director tried to establish a PMO to resolve problems with a large international project, he was let go from his position. Employee turnover is high, especially within the PM ranks - mostly driven by dissatisfaction with upper management's failure to act.
