Feedback tends to happen mainly when something goes wrong, rather than being part of a healthy and ongoing dialogue.
Mistakes are not openly discussed as learning opportunities and instead often feel hidden or quietly absorbed, which contributes to a culture where blame spreads faster than solutions. Decisions also change frequently, sometimes without clear communication, which creates confusion and makes it difficult to plan or execute consistently.
There is also very little challenge or constructive debate encouraged across teams. Silence is often interpreted as alignment, even when there are clear concerns or misalignment underneath. Over time, this leads to disengagement, and many strong performers appear burned out or eventually leave.
