People management is clumsy and inexperienced. Recent technological + market winds could make this problem space much more difficult to emerge out of.

As a bootstrapped startup we have to be vigilant about moving fast while growing the team at a responsible pace. There has been some turnover, but I truly believe we have solved that problem and are rebuilding the team with a solid foundation.
