Mostly the management doesn't want to find the root causes of the problems and just want to make some actions to pretend like they do something. People don't trust each other when they work in one team to solve the same problem. People change very often.

As with any large, established automotive corporation, decision-making can be slow and sometimes bureaucratic. Getting approval or consensus across the different global divisions can take time.
