Micromanagement, adversarial relationship between company and employees (lack of trust), authority based on fear, middle managers worried about assigning blame instead of solving problems. Huge systems being deployed with no automated testing. Call themselves "Agile" because they have whiteboards with post-its and stand ups. In the end, it's the same old waterfall process. Endless meetings.
